Dr. Bernstein's Retirement Notice

Dear Patient,  

I am writing to inform you that I will be retiring and closing my medical practice at Clinic Medical Dorval effective as of March 31, 2025.  

Until the date of my departure, I will do my utmost to find my patients a medical replacement amongst the other physicians or nurses at the clinic.  Patients who will be retained by one of these professionals or by the Clinic group collectively will receive a letter from RAMQ identifying the group, physician or other professional who has accepted to assume responsibility for their file. If you receive such a letter, you do not need to take any further action to confirm this assignment.  

Those patients who will not be automatically followed (or do not wish to continue their medical treatment at the Dorval Clinic), will become “Patients without a Family Physician/Orphaned Patients” as of March 31, 2025.  I strongly recommend that if you fall into this category, you register on the GAMF waiting list available on the Quebec.ca website or by phone as soon as possible. You may access this waiting  list at quebec.ca/sante/trouver-une-ressource/guichet-dacces-medecin-famille. 

You may also verify a phone number to call based on your home address on the government website.  You will also be able to get instructions about what you can do to access medical services while you wait to be assigned to a new Family Physician or Nurse Practitioner.  To obtain further assistance once you have registered or to make an appointment with a medical professional, you can call 811.  

Your medical file will remain in the care of Clinique Medicale Dorval/ GMF Dorval after my departure.  Should you wish to transfer this file to a physician or professional at another location, you will be able to do so with signed consent. There may be a fee associated with this service.    

I would like to take this opportunity to thank you for the confidence you have placed in me as your physician.  It has been a privilege to provide you with medical care.  I wish you well in the future.
